Custom Hat Questionnaire 
Please complete this form to provide me with details to design your Custom Hat. Once the form is completed, I will design a mockup to send over for approval before I begin painting. Hats are $40, unless you need it sooner, than a $15 rush fee will be applied. The turnaround time is 2-3 weeks + shipping. A $15 deposit holds your spot in order of production, with the remaining balance due prior to shipping.
