Hands-On Registration Form 2019 - 2020 School Year

There is a 10 student minimum for ALL hands-on programs. Please keep in mind with large groups that you may need to schedule back-to-back time slots, if your number of students exceeds the limit for the lab. The cadaver demonstration lab can accommodate up to 80 students at one time comfortably (but we can do up to 100 if necessary), and the dissection lab can accommodate 65 students at once.

One Program - $16 per student for all students participating in cadaver demonstration, pig heart dissection or sheep brain dissection. Pig Eye Dissection is $12. Teachers or chaperones do not need to pay.

Two Programs on same day - $27 total per student ($5 per student discount)

PLEASE NOTE: If requesting multiple program topics on the SAME day (i.e. Cadaver Demonstration and Pig Heart Dissection), please fill out additional information AFTER "special instructions" section on this form. This is so you do not have to send in two separate registrations. $5 discount per student if you sign up for two programs on the same day. Also, types of dissections may vary based on availability, time requests, etc, therefore, can be subject to change. Please put your PREFERRED dates/times and we will contact you should we need to adjust it. If you would like multiple program topics on DIFFERENT days, please submit a registration form for each date. Thank you!

PAYMENT POLICY : I understand by submitting this registration that I will be invoiced for 80% of the total cost, which is due immediately. For any remainder that is due, you will be billed separately on the day of the program. If the 80% is not paid immediately, and your reservation is still held (upon request), you will still be responsible for that amount on the date of the program. This means, please be aware of the number of students you are estimating to attend. You are always able to call later and add a few students if we have the space vs overestimating "just in case" and bringing far less. For example: If registering for 100 students, you will be billed for 80 students. That amount is due immediately (or another date if agreed upon with AIMS). On the day of the program, if you bring your full 100, you will be invoiced the balance. If you bring less, we will refund up to 5 students. Cancellations 30 days or more before the scheduled program will receive a full refund. Cancellations within 30 days will receive a refund, less a $100 deposit. This is to help cut down on the overestimating and no shows we get each year which greatly impact our ability to service as many schools as possible.
