2018-19 Middle School Registration Form
Middle schools must be registered and payment of dues must be postmarked by November 1, 2018.

This form must be submitted online. After the registration form is submitted, an invoice will be mailed to you immediately.

Payment must be snail mailed to the IHSFA.

Registration is only considered complete upon receipt of payment.

Registration and payment entitles you to attend, at no additional charge, the middle school sessions at the 2018 IHSFA Coaches' Conference on Saturday, September 8.

Any fines imposed because of any check returned by the bank due to insufficient funds, closed bank accounts, or any additional fines/fees, are the responsibility of the school/school corporation submitting that check in addition to the original amount for which the check was written.

IMPORTANT INFORMATION
Submission of this form certifies that the named coaches are responsible for the entry of the school in middle school tournaments. They are certified to enter your school's students for middle school tournaments. All entries submitted must be full time students in good standing at their middle school. A VALID AND PROPERLY TYPED EMAIL ADDRESS IS REQUIRED FOR THIS FORM.

Middle school sessions at the 2018 Conference take place on Saturday, September 8. Registering your team and submitting payment to the IHSFA entitles you to attend the middle school sessions at the clinic at no additional charge.
