I understand and approve my player’s participation in the Geneseo Youth Football program for the 5th through 7th grades. I will not hold any supervising Youth Football coach or the Geneseo Youth Football Board of Directors liable for any injury my player might sustain while participating in the program. I also understand that I am responsible for value of all equipment issued to my player or any part thereof, if not returned. This includes jerseys, pants, helmet and shoulder pads. *
