D6 Expression of Interest Form 
Thank you for your interest in your child/ren attending D6!

Please complete the form below and we will be in touch to confirm your child's place. D6 starts at 9.30am and runs until 11.30am. Please note children must be signed in at Reception at Sandbrook Community Primary School and collected by a parent/guardian listed below. Dates for the year can be found here. Our first session will run on Saturday 11th October 2025.

We will provide your child with breakfast, please let us know if your child has any allergies or intolerances we need to be aware of along with any medical issues.
