Due to space/timetable limitations, not all requests will be possible. PARENT/GUARDIAN APPROVAL/SIGNATURE IS NECESSARY FOR ANY CHANGES. In order to be processed, this form must be printed out (right click/print) and submitted with a signature to the Guidance office. ONLY students going into Grade 12 may request a "SPARE" and must still register for 6 courses.
