Course Change Request 2019-2020
Due to space/timetable limitations, not all requests will be possible. PARENT/GUARDIAN APPROVAL/SIGNATURE IS REQUIRED FOR ANY CHANGES. In order to be processed, this form must be printed out (right click/print) and submitted with a signature to the Guidance office. If you have questions or concerns, or you are requesting pathway changes/ multiple course changes, make an appointment with your Guidance Counsellor (http://www.colonelby.com/guidance.html).
ONLY students going into Grade 12 may request a "SPARE" and must still register for a minimum of 6 courses.
