Furkids Dog Adoption Form
🐾 Welcome to FurKids by Nagham Badawi
Firstly, we would like to thank you for considering adoption — you are truly making a difference.
Secondly, we would like to explain our adoption procedure so you are aware of each step.

Adoption Procedure
1. Application Form
Fill in the adoption form using accurate details. Please make sure all questions are answered carefully, as this helps us match you with the right dog.
2. Interview & Meet-and-Greet
If your application matches the requirements of the dog, you will be contacted via WhatsApp for a short interview.
If the interview goes well, a meeting will be arranged with one of our team members, the dog, and all your family members to observe interactions.
If your application does not match the dog’s needs, we may not be able to contact you, as our small volunteer team dedicates much of its time to ongoing rescue operations.
3. Medical Fee Contribution
Once both parties agree to proceed with the adoption, the adopter will need to pay the advance or current invoice covering the dog’s health check, microchip, vaccines, and spay/neuter.
This payment (1,000 QAR) should be made directly to the clinic as a refund for the dog’s rescue invoice.
If no clinic invoice exists for the dog, a contribution of the same amount is appreciated and also paid directly to the vet clinic.
A copy of the clinic’s invoice will be shared with you.
4. Adoption Trial
The dog will be dropped off by one of our team members for a two-week trial.
If everything goes well, the adoption will be finalized and the pet’s passport can be collected from the clinic.
5. Home Visit & Final Contract
After approximately one month, a team member will visit your home for a follow-up visit and checklist review to ensure all requirements have been met.
The adoption contract will then be signed and dated. A PDF copy will be shared with you after the visit.

Health Disclaimer
We are a group of volunteers operating with limited funds and personal contributions. We always do our best to provide healthy, well-cared-for dogs, but please note the following:
Each dog receives a CBC test, Canine 4 test, and a general veterinary check-up, but we cannot guarantee that no underlying health issues exist.
We recommend a full health check-up by your own veterinarian before finalizing the adoption.
We do not cover any boarding responsibilities or fees during the trial or after adoption.
All medical costs following adoption are the responsibility of the adopter.
Dental care is not included in the adoption process, as our resources are limited.

Adoption Fees and Remaining Medical Costs
Adoption fees contribute toward the expenses already incurred by FurKids for the dog’s rescue, care, and medical treatment prior to adoption.
However, the adopter acknowledges that certain procedures may remain pending at the time of adoption. The adopter agrees to complete and cover all remaining veterinary procedures, including but not limited to:
- Spaying or neutering (mandatory for all adopted dogs)
- Vaccination boosters
- Dental care or other follow-up treatments, if advised by the veterinarian
If the dog has not been spayed or neutered prior to adoption due to age, health, or medical reasons, the adopter must complete the procedure as soon as it is medically safe and cover the cost.
FurKids will provide access to our partner veterinary clinics at discounted rates to support adopters in completing these requirements.
