Optional Questions
The following questions are optional, you may answer as many or as few as you like! Coordinators check responses a few times a month.
Is there anything else you want to communicate to coordinators at this time? Write it here!
What material are you learning from currently?
This may be a book. a podcast, youtube channel, lived experiences, study guide, or any other material your group is using to inspire discussion and reflection.
What general updates, lessons learned, resources you'd recommend, or questions do you have for the larger SURJ Worcester community or SURJ Worcester coordinators?
At this time, does your affinity group want facilitation support around a particular piece of material or content area? This could be to help understanding, offer prompts for different and more challenging reflection, or to help with group dynamics and process (or something else!)
Clear selection
When is your affinity group meeting? If your group does not meet at a regular time/day, please let us know when next 3-6 meetings are so we can get them on shared calendar!
This is not for any kind of oversight or judgement, we're just curious as to what kind of time commitment each group has for placing new folks AND for understanding how much time groups typically need to X amount of content. Thanks!
Please indicate here if any of your affinity group members have missed more than 2 meetings in a row. First name and first initial of last name is great, thank you!
We are not tracking attendance, but tracking absences for two reasons. 1) If someone has missed 3 or more meetings in a row, we want to reach out to them 1:1 and see how they are doing. We are a community and we must take care of each other! 2) SURJ Worcester expects everyone in work groups and in general meeting to be participating in their affinity groups first and foremost. We want to be able to ensure this is happening.
Do you have space / Would you want new members at this time?
Clear selection
If you answered yes you would welcome new members, how many would be ideal? Consider Affinity Groups are most effective 5ish - 10ish members!
Who is filling this out (in case we need to clarify anything later)?
