The UH System is in the process of planning to ensure the availability of the COVID-19 vaccines to UH Students, Faculty, and Staff (for those who wish to be vaccinated). We are asking that each student, faculty, and staff member of the UH community complete this questionnaire.  It will take less than 5 minutes to complete. Thank you in advance for your patience and understanding.

Please be honest with your reporting of risk factors. This will help us to further prioritize groups to help ensure that the individuals at the highest risk receive the vaccine first. Over time, there will be enough vaccine doses to cover all who want it. However, given the complexities, timing, and allocation of the vaccine doses the State receives each week, we want to do our part to assure an equitable distribution that balances the risk of dying from COVID-19 with the need to keep essential services running.

We are collecting your primary work email address (hawaii.edu, cc.hawaii.edu, ucera.org, uhfoundation.org, or rcuh.com) address on this survey TO ENSURE ONE RESPONSE PER PERSON.  Your response will help us give appropriate guidance to the priority groups within the UH community when the vaccine becomes available. This survey is not tied to any school or employment records and is voluntary. The information will be used only for planning purposes.
