Oregon Fire Marshals Association                           Member of the Year Award Nomination Form
The Oregon Fire Marshals Association (OFMA) is excited to announce the nomination period for the 20Member of the Year Award. This award will be presented at OFMA’s Annual Business Meeting & Training Conference in May 2022.
Purpose: To recognize the outstanding contributions of an OFMA member who is active in the organization.

1. Nominee must be an active member of the OFMA; and
2. Nominee must be in good standing for at least 2 years in OFMA.

Nomination Process: OFMA members are encouraged to nominate an outstanding member who benefits OFMA in a unique way and deserves recognition. For example, the member actively recruits OFMA members; the member is actively involved in community service; the member communicates his / her commitment and involvement in OFMA activities and committees.

Award: The selected Member of the Year will be recognized at the OFMA Annual Business Meeting during the May 2022 at the Best Western Plus Agate Beach.

Format for Submission: Describe this outstanding member and the reason(s) why you have nominated him / her for the 2022 Member of the Year Award. Please provide specific examples of his / her outstanding contributions. Note: Providing specific examples can help the Awards Committee better understand the person you are nominating.

Deadline: All nominations must be received by 11:59 p.m. on Sunday, May 6, 2022.

Selection of the Award Recipient: The OFMA Awards Committee will determine the award winner.
