This form is for: Youth Of Unity Sponsors attending 2018- 2019 school year Regional Events.

All Youth of Unity attendance at regional and local events is through individual chapters. To finish your registration - you must sign and turn your forms into your chapter, Understand  your typed name on this registration will serve as your signature to the region. All participants must be able to honor heart agreements/Code of Ethics, function in a group setting and MUST attend the entire weekend from check-in thru Closing Ceremony.
