Westside Community Schools Report of Concern Form
This form may be used by any Westside staff member, parent, community member or applicant to report a concern about harassment, discrimination or retaliation.

If you have questions about filling out this form or need assistance for any reason contact the Assistant Superintendent for Human Resources/Lead Title IX Coordinator or the Coordinator for Student Services/Title IX Coordinator (both at 402-390-2100) or your building principal.

Upon submission, this form will be sent to the Assistant Superintendent for Human Resources and the Coordinator of Student Services, one or both of whom will investigate the reported concern(s). If the report involves discrimination based on a disability, we will also notify the District's ADA/504 Coordinator.

IMPORTANT: This account is not monitored 24/7. If you have any emergency or if you feel you or any other person may be in danger, call 911 and make a report immediately.
