This request form is designed to help support the resource needs of our community, like volunteers, supplies, etc. For funding requests please complete our Enrichment Grant Application

Please Note
PTO requests are reviewed throughout the year. All new submissions will be added to the agenda of the next monthly meeting (taking place every 3rd Wednesday). For this reason, if you wish for your request to be added to the next available meeting's agenda, be sure to submit your request by the 3rd Monday of the month. Otherwise, your request will be added to the agenda of the next PTO meeting. All requests require Board review and approval. Please plan accordingly.

Timing your Requests
Please submit your requests as soon as you are aware of the need. We try to be as accomodating as possible, but we would ask for a minimum of three weeks ahead of the need for which this request is being made. The shorter the time between the request submission and the request date decreases the liklihood of the PTO being able to assist in the execution of the request.

One Final Note
When you submit this Request Form, you are requesting support from a limited pool of resources consisting of hardworking volunteer parents, students, community members, alumni, and supporters of Mountain View Elementary. We expect that all requestors uphold and respect this fact. We expect good stewardship of the support granted with the highest level of transparency possible. It is our hope that, if your request is fulfilled, you will conduct this event, activity, etc. in such a way that we can provide a successful report to our PTO board and committee, illustrating the value of the fulfilled request.
