2026-2027 CKYO Absence/Tardy Reporting Form
When possible, please complete and submit your Absence/Tardy Form no later than 2 weeks prior to a KNOWN absence (school events, trips, etc.).  This helps the CKYO staff plan around large absences at one rehearsal. Check your school calendars and submit any upcoming absences well in advance. You may also use this form to report any absences as they arise within 24 hours before or after the absence (Illnesse. You will receive an email to confirm your submission and you are also able to edit any absence requests that you have submitted. Please check Spam or Promotions folders if you do not receive your email confirmation right away - and make sure your email is entered correctly.
