I will not hold Seattle School District, Chief Sealth Int. High School, Chief Sealth Cheerleading Camp or cheerleaders/coaches responsible for any illness or injury to my child. This will also certify that I have insurance to cover any injury that might occur during participation and/or travel to camp. I also give Chief Sealth cheerleaders permission to take pictures of my child during camp and use them for high school publications/publicity.

You will need to provide your own transportation to and from the camp. Please pick up your students no later than 8:15pm.

If payments are not received on time, you will NOT receive a t-shirt for the half time performance. The cost is $60 and FULL PAYMENT is required. NON-REFUNDABLE.
