Temporary Transportation Changes
This form is ONLY for a child that needs to have their transportation changed for one day. The cutoff time for notifying the office of a temporary change is 1:30PM. If there is a permanent change needed please click the link below, print the form and bring it to the office. We also have printed copies in our office as well:

Phone number of Parent/ Guardian requesting change *
Temporary Change requested (Ex: bus change, walk, parent pickup) *
If change to new Bus stop please list Bus number and stop requested.
If child being picked up please list name of person. (if they are not on our approved list please have them come to office to show their ID)
A copy of your responses will be emailed to the address you provided.
