As parent or legal guardian, I hereby give permission for my child to participate in After School Art Clubs conducted by Art Nest Creative Studio. By signing below, I understand that I, and the child in my care, are participating in a programme, which includes certain risks in outdoor activities. I, and the child in my care, are voluntarily participating in this programme and hold harmless Art Nest Creative Studio and its agents from all responsibilities of personal injury. I hereby waive any and all claims against Art Nest Creative Studio and its board members, members, employees, and volunteers for any damages, injuries, loss or liability, which may occur, during my child’s participation in this programme. My child has permission to engage in all prescribed activities except as noted. Art class activities may be documented with video and still photos and images of my child may be used in future public relations and advertising. I give Art Nest Creative Studio permission to use those images and video for the purposes of advertising and public relations only. By checking 'Yes', I agree. *
