- Incoming Grade 8 students who want to enroll in "Algebra 1" or "Geometry" instead of "Math 8" (the grade-level Grade 8 math course).
- Incoming Grade 9 students who want to enroll in "Geometry" or "Algebra 2/Trig (accelerated)" instead of "Algebra 1" (the grade-level Grade 9 math course).
NOTE: If you are comfortable with your child enrolling in the "regular" grade-level course listed above for his/her grade level, you do not need to complete the form below, as there is no need for your child to take a math course placement test; your child will be enrolled in his/her grade-level math course (Math 6 for incoming Grade 6 students, Math 7 for incoming Grade 7 students, Math 8 for incoming Grade 8 students, and Algebra 1 for incoming Grade 9 students).
If you would like your child to enroll in an accelerated math course as indicated above, please complete this entire form and click "SUBMIT". Please complete a separate form for each child who needs to take a placement test.
In addition to the auto-generated notice you will see after clicking "SUBMIT" confirming that you have completed the form, you will also receive a separate confirmation email within 3 days from Dave Chamberlain, Capistrano USD Mathematics Teacher on Special Assignment. Dave's email will provide you with more specific the details of your child's placement test, and will address any questions/comments that you may have included in the form below.
