Private School Student Registration for Summer 2017 EOC Assessments - Polk County

Please complete the attached form in order to register for our Summer 2017 administration of the following EOC Assessments - Algebra 1, Algebra 2, Biology, Civics (middle school), Geometry, and US History (high school).

    Private School Student Information Registration

    Dr. Tony Bennett, former Commissioner of Education for the State of Florida, released a memorandum on April 15, 2013, regarding private school student participation in Florida End-of-Course (EOC) Assessments for students who plan to enter public schools at some point in their educational careers. The Polk County School Board will provide this service to private school students during the state’s summer EOC administration window, July 10-13, 2017. In order to take advantage of this opportunity, private school students will need to complete this Online Registration Form by June 16, 2017. Confirmation and additional information will be communicated back to the family via provided email address/phone number by June 23, 2017. Completion of the registration form will serve as the student’s intent for testing and provide us with the necessary information to enter the student into the state testing system. No late registration is available for these assessments. For the NGSSS EOC exams (Biology, Civics, US History), students are required to sit for at least 80 minutes of each assessment and will not be permitted to leave (even if completed and submitted) prior to this time limit. For most students, each EOC takes approximately 3 hours to complete; however, students may continue testing until the end of the school day in order to complete the assessment. Snacks or lunch will not be provided. However, a 30-minute break is provided for a student to eat after the state-required 160-minute sitting for these NGSSS EOC assessments. The FSA EOC exams (Algebra 1, Geometry, Algebra 2) involve two testing sessions over two days. These sessions are 90 minutes each, but the student may have up to 3 ½ hours each day. Please note that the Florida Department of Education typically allows students to take both sessions in one day for the summer administration only.     Summer EOC Testing Window

    All testing will take place between July 10 - 13, 2017. See below for the testing schedule. The Testing Coordinator at your testing site will contact you via email or phone on or before July 7th to verify registration and provide you with additional information, such as testing procedures and specific testing times.  July 11 & 12 (2-day tests) - FSA Algebra 1 EOC, FSA Geometry EOC, FSA Algebra 2 EOC  July 13 - Biology EOC, Civics EOC, US History EOC
