AHF FC Pan Disability Football
Welcome to AHF FC's pan disability football registration form. To register, please complete all required fields ensuring the correct details are inputted. The pan disability football is open to boys 7 + and girls 7 - 11 years and will operate every Saturday at Blackburn Powerleague Soccerdome.

The coaching sessions will be delivered by qualified and experienced disability football coaches in a safe, friendly and fun environment. As well as the football coaching sessions, the participants will be involved in games and tournaments, fun days, parties and club outings.

I consider my child capable of taking part in coaching, games and competitions. I have provided all required information about their impairment and consent, in the event of any illness/accident, a suitably qualified person can administer any necessary treatment to my child.

I also accept that there are risks associated with participating in football and sports and AHF FC will take every precaution to minimise the risks.

I give consent to Abu Hanifah Foundation to take photographs of my child to use for the purpose of promoting health, physical exercise, football and participation.
