Hall of Fame nomination form
The following form is used to submit a nomination for the St. Anthony Village High School alumni Hall of Fame. Please make sure to provide as much detail as possible to ensure that your nominee is considered.

All nominations are due no later than Jan. 31, 2020. Once nominations are received, they will be reviewed by the Alumni Board, with a final decision made by members of the Hall of Fame. Awards will be presented on April 26, 2020, following the matinee of the POPS band concert.
