This report will be logged and provided to the Superintendent of Schools along with the appropriate administrative team members. Safety Reporting Form responses are not monitored 24/7.

This form is anonymous, but your contact information may be provided if you choose. Any contact details will allow us to follow up for clarification and a resolution if needed.
