School Registration
To simplify the registration process all the paperwork you need to register your student is in this form. If you have multiple student you will need to fill out this form for each student.

You will also need to provide the following information.
- Birth Certificate
- Proof of Residence
- Proof of guardianship (If you are not the parent of the student.)

If your student is registering in Public school for the first time you will need to also provide the following records.
- Immunization Records
- Physical Examination
- Lead Screening
