Plank OPL card info for digital services
Completing this form will allow your child to have access to the online digital resources offered by the Oswego Public Library.  Upon completion, your child's school librarian, Dr.Meyer,  will contact you with a library card number and access instructions.  Be sure to complete an application for every child in your household who attends Plank Junior High and wishes to receive access.  Please contact Dr. Meyer with any questions.  630-636-9403 or email at smeyer@sd308.org
