This form is intended for Current EPLI Level 1 Trainers of START Content to report their training activity to START and their respective RCN.

Trainers are instructed to report all trainings they conducted using START content/materials.

This form should be submitted twice a year:
Mid-year deadline: February 15 (Note: this deadline has been extended)
End of year deadline: July 14

The data from this report will be shared with the RCN after each of these deadlines.

Please note: Trainers will be designated as Inactive if more than two years have passed without a report of a training. Inactive Trainers are required to reactivate according to the then-appropriate START reactivation process.
