IMPORTANT NOTE: I will start working a second job at General Dynamics Electric Boat and will be require to be there full time...but this does not mean I am leaving/ quitting the shop. This only means that I will have a change of schedule and will be in the shop a lot less during the week. Orientation is on July 9th and 10th...from there I will be assigned 1st or 2nd shift and will be able to figure out my schedule for the shop!

This form is to be filled out for one person only. If your looking to book for multiple people for one appointment, please have everyone fill out their own form and mention in the description that your looking to book together.
If you're the parent/ guardian for the child getting the piercing, please fill out this form with their information and note your information second!
Age Requirements for each piercing:
14+: Nose, Tragus, Rook, Cartilage/ Helix, Conch, Lobes, Hidden Helix
16+: Navel
18+: Eyebrow, Daith, Industrial, Septum, Nipples
When booking you're allowed 3 piercings max!!
If you are younger than the age of 18 years old you must have the following to book:
- Birth Certificate
- ID ( Passport, School ID, Driving Permit are allowed )
- Parent/ Guardian must be present and will be asked for their License
Piercings I DO NOT do:
- Oral Piercing
- Dermal Piercings
- Genital Piercings
Please keep in mind that everyone's anatomy is different and some piercings may not be possible to be done. If you're unsure about your anatomy working for the piercing(s) you want, book in for a consultation first! Consults are FREE!!
At our shop we pierce with Titanium and everything is sterilized before piercing with. If your looking to switch your jewelry out to something new, but need assistance, the jewelry CANNOT come from outside of our shop as we are not sure what kind of quality it is or where it came from. Also all piercings are done with a hollow needle NOT a piercing gun!
Looking to do just a piercing swap? Select none for each piercing section and fill out the rest of the form!
Piercing swaps are $20 for the service fee (per piercing) and the additional cost will depend on the the jewelry you choose. All jewelry starts off at $25 for Titanium jewelry and roughly $50 - $60 for 14K Gold jewelry.
If your looking to change multiple piercings out at once, please let me know and I will give you a deal for the service fee!

Bringing a guest is limited to one other person. Anyone else that comes along will have to wait in the lobby as there is limited space within the booth.
Email responses- You will get an email response within 2-4 days after submission. Be sure to check your spam folder in case it finds itself there. After I response, I will give a week to hear back from you. If I don't hear back from you within that time, I will delete the form as I will believe you're no longer interested. If you have any questions for me, please let me know!
Deposits- A deposit must be made in order to be scheduled in, and can be put on hold for a day or two. If you don't put a deposit down within the time given, it will be removed from the schedule. Deposit will be half of the piercing cost of each piercing you are requesting. Deposits will also come off the total cost after the piercings are done.
Canceling or Rescheduling an appointment- Give me or the shop a 48 hour notice if you need to reschedule/ cancel your appointment so that your deposit can be honored. Anything less than 48 hours and/ or being 15 or more minutes late to your appointment can result to the deposit to be forfeited. Please also be mindful of how many times in a row you request to reschedule an appointment, this may also lead to putting another deposit down. ALL deposits made are non-refundable and go through the shop!!
