Time Frame: 2025-2026 Academic Year

The student named below has my permission to participate in the Middletown Township Public Schools’ Senior Flex program. This program allows students to take less than a full schedule in grade 12. Participants in this program are not guaranteed a parking spot at their high school, however, students in the program are responsible for their own transportation. The Senior Flex schedule does not guarantee elective requests or for the schedule to fall in any certain way.  I understand and acknowledge that if my child receives a parking spot, that in choosing to provide his/her own transportation, this student is taking upon himself/herself all rights and obligations of an independent driver on public roads, including the possibility of accidents, the incurring of liability, assumption of risk including the condition of roads and the automobile being used, and the obligation to have insurance. As such, on behalf of this student, I hereby relieve the Middletown Township Public School District of all responsibility for this independent transportation, waive any and all claims whatsoever against the Middletown Board of Education, its agents, employees and representatives arising out of such transportation, and agree to indemnify and hold harmless the Board, its agents and representatives for any claims arising out of this student’s independent transportation to this program.

Eligibility for this program is not a guarantee of approval to participate.
