Prerequisites:
  1. Rescue diver certified
  2. FCOA Coach certified

(You only need to provide the following if your instructor specifically has asked you for it. If not, then move on to the next Section)

1. Medical clearance. 
2. ID to proof your older then 18
3. Open Water certification
4. Advanced Certification
5. Rescue Certification
6. Medic First Aid Training proof
7. Oxygen Provider proof
8. Name the Instructor that passed you for the followoing Divemaster topics

9. Proof of 60  logged dives
10. Proof of passing the Divemaster Stamina Test (must be 15 or more to pass)
11. Note down the names or member numbers of the 5 FCOA members you certified as FCOA freedivers
12. Note the names of the four students who's DSD's you conducted under Instructor observation. 
13. Note 2 names of students whose open water course you have participated in. 
14. Note 2 names of students who's continued courses you participated in. 
15. Name the Instructor who passed you on your 20 Divemaster skills. 
16. Name the Instructor who passed you for the Rescue drills for the Divemaster course. (Additional to the Rescue course)
17. Upload a link to your emergency plan
18. Upload a link to a dive map you made

8. Supply proof like a logbook entry signed by your advanced instructor that you now have Diver Master level of knowledge on the following topics.

Share a picture link of a logbook entry or other document that proofs this. 


9. Upload a picture link of proof that you have logged more then 60 dives

10. Upload proof of having passed the Divemaster Stamina test. Either a logbook entry signed by your instructor or video proof of you completing the test. 

11. Note down the names or member numbers of the 5 FCOA members you certified as FCOA freedivers

12. Note the names of the four students who's DSD's you conducted under the direct supervision of which Instructor? Also add picture links of your 4 logbook entries that proof this. 
13. Note 2 names of students who's open water course you have participated in. The name of the instructor that was in charge. And the picture links of your logbook entries related to those dives. 

14. Note 2 names of students who's continued courses you participated in. Who was the instructor in charge, what continued course they did. Also add picture links of your relevant logbook entries. 
15. Name the Instructor who passed you on your 22 Divemaster skills. And upload a picture link of his declaration. Or for Remote Verfication Training, upload video links of you performing the skills at demonstration style, slow, clear and exaggerated.

16. Name the Instructor who passed you for the Rescue drills for the Divemaster course. (Additional to the Rescue course) or share relevant video link performances as proof.

17. Upload a link to your emergency plan
