Re-Admission Request
Note: This application is to be used only if you have previously attended and completed at least one semester as a graduate student at the CUNY Graduate School of Public Health and Health Policy and would like to be re-admitted. Students who matriculated over five years ago are required to complete the program curriculum as it is exists at the time of re-admission. Courses previously completed by the student will be individually evaluated to determine their applicability to the new curriculum.

There is a one-time fee of $20 that will be added to your tuition bill at the time of registration. Please do not send the application fee when filing your application.
