Once this form is filled out, a Service Desk Ticket will be started for the Technology Department. A Technician will be in communication with the Teacher and other building leaders once the issue is diagnosed and next steps can then be determined (i.e., repair costs, long term solution, notice to Principals and Deans, etc.)
