Transcript Request Form
    Ontario Student Transcripts (OST) are records of student achievement and are required for post-secondary institutions, employers and for a number of other purposes.

- Please complete the Transcript Request Form.
- Transcript requests are not processed on the same day of request.
- Transcripts will be sent directly to the postsecondary institution unless otherwise indicated.
- We will acknowledge receipt of the request by email.
- You will be advised once the transcript has been sent to the institution.

* If you graduated more than 1 year ago, there is a $25.00 fee for the first transcript. Additional transcripts are $5.00 each. Payment can be made online at www.school-day.com. Create an account and add OLQW as your school. Payment can be located under the "Products" tab. Transcripts will only be processed once payment has been made.

Statement of Privacy of Information
As part of the transcript protocol, the YCDSB may be required to send transcripts to third parties to support the completion of scholarship applications, or applications to Post-Secondary institutions outside of Ontario. Please read each of the following carefully as we need you to provide us with your consent before we can complete your request.
This information will be sent electronically, through email, to the registrar address provided by the Requestor. *
As part of your request, you will need to authorize the sharing of Personal Information for the purposes set out in this Consent. *
All Personal Information shared will be kept confidential by YCDSB and the YCDSB shall take security measures reasonably necessary for the protection of the information against unauthorized release or disclosure. *
Understand that you can refuse to provide this consent or withdraw consent, however, this means that we will be unable to meet the requirements of a scholarship application or application to a Post-Secondary Institution outside of Ontario. *
