The purpose of this form is to identify the most common injuries and discomforts experienced by mechanics. This information will help me create a tailored workout program specifically designed for your profession.

As a token of appreciation for your participation, I will provide you with a free copy of the program once I have gathered sufficient data to develop it.

If you are suffering from a serious injury, you could still benefit from this program by working unaffected areas of your body and preventing more injuries, but please refrain from doing the exercises that work the injured area of your body. Refer to a health professional instead.
