If a specific course and institution are listed on the transfer credit website (transfer.msu.edu), than you are NOT required to fill out this form and you are not required to see an advisor.  Please check the website before submitting this form.  For existing equivalencies, any changes to the equivalency must be enacted in a future semester (ex: we can't go back and change how a course already evaluated would come in from a previous semester).
