Foresthill Union School District California Healthy Kids Survey Consent Form
Your 5th or 6th grader has been asked to participate in Foresthill Union School District's California Healthy Kids Survey.  Responses to this survey help guide district budgeting, hiring, and instructional planning and helps your child's teachers and administrators better meet your student's needs.  This survey is 100% anonymous and voluntary, questions in the survey that the student may not have an answer for can be skipped. The survey does require parent permission.  A print version of this form is available in the FDS office.
