Carrie Steele-Pitts Home, Inc, The United Way, and DFCS proudly presents TAGS Brave's 2026 Summer Camp. The information provided on this form will help ensure that eligible youth will both benefit and enjoy the summer camp experience focusing on "LEVELING UP" the TAGS Brave way!

Note: To qualify for TAGS Brave Camp 2026, each youth must qualify for benefit(s) listed in question 1. If you do not meet eligibility requirements listed below, please do not proceed with completing the form and contact us directly at tags.csph@gmail.com.

Please complete this form in its entirety and submit electronically confirming your youth's interest in attending. Be reminded that this is an interest form and does not guarantee attendance as spots are limited. Please submit 1 form for each eligible youth and please do not duplicate entries.
