This form MUST be used for each and every facility request for each and every event in any district owned building.  ALL events must be Board of Education approved without exception.  So please plan accordingly as only the Board of Education may authorize use of the facility.  Minimum of one months notice must be given for an event to obtain BOE approval (This includes internal events as well).
