This form is for parents who will pick up their student AFTER the student has completed all obligations. Students are not allowed to leave until all obligations are complete as designated by the director.

Please read all sections before signing acknowledgement. Complete one form per student. After completing the form, parents/guardians must also meet with a director or chaperone and show a valid ID before leaving with their student.

Please read and check that you acknowledge the following rules: *

I understand the Designated Person must report in-person and present photo ID. The designated pick up person will sign out the student with the Head Bus Chaperone or a director prior to bus departure. THE STUDENT WILL ONLY BE ALLOWED TO LEAVE WITH THE DESIGNATED PICK-UP PERSON.

I understand that if the student listed above has not been picked up when the Marching Band is ready to leave, they will ride the bus back to Bentonville West HS. If the designated pick-up person doesn’t sign out the student before leaving, the student will NOT be allowed to go directly home from future events.

I understand I am responsible for the return of the marching band uniform in a timely manner and will not try to wash the uniform without proper washing instructions. (Washing without instructions WILL damage the uniform.)

I understand and acknowledge the policies and conditions for the early release of the student. By signing this form, you will release Bentonville West from transporting your student back to campus at the conclusion of the event and release all liability from the moment the designated person signs your child out.
