Bike Rodeo Permission Form- 9/12/2018
Each year at Olive, we require that all 3rd, 4th and 5th graders who would like to ride their bike to school
(and have not participated in the past) participate in the Olive-Mary Stitt PTA Bike Rodeo. This rodeo is
an introduction to biking rules and an opportunity for the children’s bikes to be checked for safety.

Helmets​ ​are​ ​a​ ​major​ ​contributor​ ​to​ ​safety​ ​and​ ​are​ ​required​ ​for​ ​the​ ​rodeo​ (Please see the helmet
fitting guide at www.olivepta.com to ensure that your child has a properly fitting helmet).
Children may ride their bikes to school on the day of the Bike Rodeo and any day after their successful
completion of the Bike Rodeo. Subsequent to the Bike Rodeo, students must continue to follow the school
policy of walking bikes on school grounds and riding in a safe and responsible manner, or riding privileges
will be taken away.

While we are happy to share safety and skill information with the children, it is ultimately the parents who
need to ensure their children’s biking skills are adequate. Thanks to the efforts of our OMS PTA, we can
encourage safe and responsible biking.

By filling out this form, you are giving permission for my child/children to participate in
the Bike Rodeo at Olive-Mary Stitt School on Wednesday, September 12th, 2018, after school until 4:45
p.m. MUST BE SUBMITTED BY TUESDAY, SEPTEMBER 11, 2018.

NOTICE: This event is solely sponsored by the PTA. School staff may not be present and have no
responsibility for the event. Please direct all inquiries or concerns to the sponsoring PTA.
