This form must be completed by a legal guardian.
You do not need to complete this form if you are already a member.
Summer membership entitles the player to:
- Attend summer hockey activities between 1st April and 31st August, subject to an additional fee.
New members will receive their first 3 sessions
free, after which you will be charged club membership fees. You should
contact the club if you do not want to continue as a member of the club.
By completing this membership form you confirm that the player will be bound by the player code of conduct and the club constitution. Failure to follow club regulations may result in the cancellation of membership following the process detailed in the constitution
