Flemington DIY Grievance Form
Complaints concerning discrimination on the basis of disability by Flemington DIY may be sent through this Google Form. Alternative means of filing complaints will be made available for persons with disabilities, so please email info@flemingtondiy.org for more information. The complaint should be submitted by the complainant and/or their designee as soon as possible but no later than 30 days after the alleged violation. We will contact the complainant within 15 calendar days after receipt of the complaint to discuss the complaint and will respond in writing within 30 days of the discussion.

The grievance will be dismissed without further investigation if it is determined that the grievance:

• is unclear or incomplete to the extent that there is insufficient information on which to commence an investigation, and the complainant did not provide the requested clarifying information within 15 calendar days of the request.

• the allegations raised by the complaint have already been resolved.

If the grievance is dismissed following the initial assessment, the complainant will be notified in writing within 15 calendar days of this determination.

The time frames in the above procedure shall be adhered to unless Flemington DIY allows an extension of the times due to unusual or unforeseen circumstances. In the event a time frame is extended, Flemington DIY shall notify the complainant of the extension.
