5th Grade Planning for next year

Dear 5th Grade Parents/Guardians:

     We are planning for the transition of our 5th grade students to the Putnam Avenue Upper School. In order to be effective, it is important to identify those students who will not be attending the Putnam Ave Upper School for the 2025-2026 school year. If your child is going to the Upper School, records will automatically be sent over. 

     I would appreciate it if parents/guardians of students who are leaving the district (moving or planning on attending a school other than PAUS) take a few moments and fill out the form below. Please return it to the Main Office by Friday, May 16, 2025, so we can properly prepare records for transfer.

     Thank you for your cooperation, this information is very important to ensure your child's school record is transferred appropriately to their next school.
