Release of Liability Waiver: I affirm that I am in adequate physical and mental health to participate in the activities and assume full responsibility for consulting with a physician before participation. I agree to indemnify and hold harmless the teachers and affiliates from any losses or liabilities incurred as a result of my participation in the activities. I acknowledge that I am choosing to attend these events at my own free will and assume all risks associated with the activities. I hereby waive any right to Sarah Zuber and Darci Fulcher for any financial or other responsibility in the event of any occurrence during the activities.
