Joann Burns Memorial Scholarship Application for Health and Science Education
This scholarship is available to any student accepted into or enrolled in a full-time program leading to a career in the Health and/or Science field. Priority will be given to individuals who have:
• Lost a parent, sibling, aunt, uncle or grandparent to cancer
• Attended a Klamath basin high school, public or private
• Volunteered 50 hours of community service and/or show proof of hours spent aiding an individual battling cancer

This scholarship may be used for any school-related expenses such as tuition, books, lab fees, etc. The awarding committee will consider only applicants with a cumulative G.P.A. of at least 2.75, with preference being given to 3.0 and above.

It is encouraged that you pre-write all narrative entries in word to cut and paste into google form application. All questions are required to be answered.

Instructions: Application Google Form and the three letters of recommendation google forms are due by 12:00 p.m. (noon) Friday, May 17, 2019.

1. Complete this application form. Including the following:
a. Upload PDF copies of your high school or college transcript to date (official transcripts will be requested upon selection).
b. A 300-500 word essay about yourself, including hobbies, plans and aspirations, factors which have favorably or adversely influenced your life, health, reason for desiring to attend nursing school, and any other information about yourself which you think pertinent.

SEPARATELY: Three letters of recommendation and recommendation form are required to be submitted using a separate googleform. These references should be from individuals who know you well. These may include a businessman or businesswoman, minister, professor under whom you have taken courses this year, or present employer. All components of the application process are due Friday May 17, 2019 by 12:00 pm (noon).
