Please complete the following form and agree to the participation consents.  If there is anything not mentioned in this packet that is important for us to know about your student, please let us know!  We believe in creating a safe, FUN, creative, family-friendly environment at Whole Steps Creative Arts Center.  Parents are always welcome to engage with staff, peek their head in to see their child enjoying our spaces, and ask questions!
