Seminar Attendance Form
This form serves as the formal attendance record for:

(1) Undergraduates attending the ICM Distinguished Seminars as a requirement for the CM Minor

(2) Students enrolled in Distinguished Seminars in Computational Medicine (EN.580.736/737) or Advanced Seminars in Computational Medicine (EN.580.735)

Seminar attendance is two-fold. Please be sure to:

(1) Attend the seminar synchronously (live) and remain on the Zoom call for the duration of the seminar

(2) Complete this form using your JHU email for every seminar you attend
