Bus Service Agreement 2026-2027
Forms are due by May 15, 2026.  Routes will be finalized after that date and additional students can be added as space is available.  Please fill out one form for each student.

The cost of the service is $900 per student, per year (round trip). For one-way service, the fee is $475 per student, per year (one way, A.M or P.M.).

The remaining balance after the $200 deposit may be paid in installments corresponding with your tuition payment plan.

Time spent on the school bus is considered an extension of the school day. All rules and policies as specified in the student handbook will be expected to be followed while on the bus. Failure to follow this policy, along with the Miller Transportation School Bus Rules, may result in suspension or expulsion from the bus.

In consideration for making arrangements for this service by DeSales High School and Mercy Academy, I hereby release and save harmless DeSales High School, Mercy Academy and Miller Transportation and any and all employees of the school from any and all liability for any injuries, loss or other claims arising out of or resulting from this service.

The undersigned parent/guardian agrees to accept all responsibility for the risks, conditions and hazards which may occur whether or not they are now known.

By signing this acknowledgement and assumption of risk and release as the parent/guardian, I am consenting to my son/daughter’s participation and acknowledge that I understand any and all risk, whether known or unknown, is expressly assumed by me and all claims, whether known or unknown, are expressly waived in advance.
