We ask that you email a copy of your matching application to smathleticboosters@gmail.com so we may track and confirm your matched gift upon receipt.

Note: The total sum of your donation (your contribution + your employer's match amount) applies to your donor level. Your name will appear on the donor plaque; your name and the name of your matching organization will be acknowledged on the SMHS Boosters' website. 
For Bearcat Fan(atic) and Team Donors: your name will appear on a plaque listing all donors for your contribution level.
