This form is to re-enroll your student into the OISP program.

Dear Families, 

You have completed the enrollment form for one or more of your children to attend the Online Independent Study Program. The following requirements are included in this agreement. Place a check in each YES box to indicate that you understand and agree to the enrollment requirements.

1. I have reviewed the Online Independent Study Program information section. I understand the information included in this section, and I choose to enroll my child(ren) in the ABC Online Independent Study Program.

2. Families requesting to re-enroll at their current school must follow the regular enrollment process for in-person school. Admittance at a regular in-person school is based on space availability. If space is not available, I understand that my child(ren)’s name will be placed on a waitlist. I understand that until space becomes available, my child(ren) will be offered another school in ABC.

3. I understand that upon enrollment in the Online Independent Study Program my child(ren) is not guaranteed a space at their current school.

4. I understand that upon enrollment in the Online Independent Study Program my child(ren) will no longer access any programs/support offered at the school from which they are no longer enrolled. Instead, resources/support will be offered through the Online Independent Study Program.

5. I understand that upon enrollment in the Online Independent Study Program my child(ren) must follow the District’s re-enrollment process to return to in-person school for the next school year.
