I, the undersigned, as the parent or legal guardian of the children listed above do hereby give my full consent and approval for my child to participate in activities at the Brookside Golf and Country Club- Kidzone. I understand that there are certain risks of play that may result in damages and injuries in the participation of my child during these activities and I hereby accept these risks on behalf of my child. I hereby certify that my child is fully capable of participating in play activities and that my child is healthy and has no unknown/hidden physical or mental disabilities or infirmities that would restrict his/her full participation in such activities. 
I hereby, for myself and on behalf of my child, agree to save and hold harmless and fully indemnify Brookside Golf and Country Club- Kidzone Management and Staff, on the account that all normal safety procedures have been taken on behalf of Management and Staff, from any and all liability for any personal injury or injury to any third-party child resulting from my child’s actions/participation in the above-mentioned activities. I hereby release Brookside Golf and Country Club- Kidzone and its entire staff from any and all liability, on the account that all normal safety procedures have been taken on behalf of Management and Staff, for any injuries that my child may sustain as a result of any activities taken place at the above-mentioned locations during the designated Kidzone hours for the duration of my child’s time at Brookside Golf and Country Club- Kidzone.

Required
KidZone is not a licensed child care center. Programs such as ours, where parents remain on property, are not required to be licensed by the state of Ohio. Because we are not licensed, there are limits placed on the types of service we can provide.

Limit of 2.5 hours is a state mandated rule parents must remain on site, and our staff must have means of contact.

Our late fee policy is a charge of $20 per child if past the 2.5 hour limit and/or past the closing time of KidZone.

Rules and Policies:

Be respectful of staff and other children. No yelling or swearing. No physical aggression. Please leave personal toys at home (KidZone Staff is not responsible for lost or broken toys). No exploding toys, toy weapons, or toys that simulate weapons. Any violation can result in a suspension of privilege. 

If your child has broken KidZone property you will be held financially responsible for replacement and your child will not be permitted in KidZone for 7 days.

Guest Policy:

Guests of current members are welcome to use KidZone. Guest fees will be paid by the Brookside member who brought the guest. All guest children must abide by all KidZone rules and are subject to the same rules as our members.

Sick Child Policy:

Fevers: Any temperature above 99.1 degrees without medication may not enter KidZone. A child may return after they are fever-free for 24 hours without the use of fever-reducing medicine.

Vomiting: A child must be 24 hours without vomiting to return to KidZone.

Diarrhea: A child must be 24 hours without diarrhea/loose stool in order to return to KidZone.

Lice: A child who has found live or dead lice may not enter KidZone for a minimum of 3 days post lice treatment. 

If your child has been sent home from KidZone or school/daycare they may not be in KidZone until the 24 hour mark free from all symptoms without medication. 

For all other illnesses please follow your doctor's directives for return to school protocols

Diapering/Potty Training Policy:

All children who are in diapers/pull ups are required to provide a spare diaper and wipes as well as a change of clothes at every drop off as we do not generally have  spares on hand. 

We do not work on potty training however we will work with your child to ask and offer a bathroom break/reminder during their time in KidZone. Please also bring a spare set of clothes and underwear at each visit.

Diaper bags and baby bottles/formula/sippy cups/water bottles are welcome, please have them labeled with your child's name on them.

Reservations:

KidZone reservations are available and strongly encouraged for all times that KidZone is open. If we have reached capacity reservations will take priority followed by walk-ins if space allows. If KidZone is full we will start a wait-list and call when space allows. If you are a no show and do not cancel your reservation you will be charged 50% of the pay per use rate after 2 missed reservations.

Tweens Only:

Children between the ages of 11-14 years old may come and go freely up to 3 times per the 2.5 hour time limit. And may leave on thier own.

Infants Only:

We welcome infants ages 10 weeks- 6 months for up to 1.5 hours per shift and 6 months-1 year for up to 2 hours per shift. Infant rate is $16/hour and a reservation is required a minimum of 24 hours prior and we can only accept up to 3 children under the age of 1 at a time.
