This year too, we’d like to strengthen each other so we can grow closer as a team. And this way achieve optimal results together. Attached you will find the feedback form. We feel it’s important that everybody gives feedback on each other, because we think it will help us all grow into persons we enjoy working with.  

Fill out the form anonymously. It’s up to the receiver to do something with the feedback or not. Please fill out the form for seven co-workers. You can choose what the feedback is about. For some co-workers it may not be possible to offer feedback regarding the content of their work. But you may also write something about their attitude in the workspace, willingness to brainstorm with others, or their contribution towards the general atmosphere within the team. Let’s contribute to the personal growth of each other, so feel free to write anything that comes to mind. [Date] is the deadline, and after that everybody will receive his or her feedback. On [date] we’ll discuss all feedback together, and end the day with drinks.
